Transition Services

Transition services are activities that prepare students with disabilities to move from school to post-school life.  The activities must be based on the student’s needs, preferences, and interests, and shall include needed activities in the following areas:

  1. Instruction 
  2. Related services
  3. Community experiences
  4. Development of employment and other post-school adult living objectives
  5. Acquisition of daily living skills (when appropriate)
  6. Functional vocational evaluation


It's never too early for parents to think about transition. Transition plans should actually drive the IEP goals so that students are prepared to the fullest extent possible.

College Programs for Students with Disabilities  is a list of programs across Texas that offer programs specific to students with cognitive disabilities that allow students to accomplish goals leading to a variety of accomplishments ranging from certifications to independent living to work experiences. ThinkCollege.net also provides nationwide programs for these students listed by state. 

The Texas Transition and Employment Guide (Spanish version of the guide) provides resources to help young people and their parents prepare for a successful transition toward education, employment, military, or independent living. This guide is a living, working online tool that stays current with input from legislation, parents, students, and other stakeholders.
